Have a health insurance claim that you need assistance with? No problem! There are two steps that need to be taken in order for us to submit a claim on your behalf:
- Please fill in the required form fields on the right of this page, including a detailed explanation of your claim issue. We also ask that any supporting documentation, such as EOB (Explanation of Benefits), billing statements, etc. be submitted to us for our use in resolving the claim in a timely manner.
We will need your permission to speak to the insurance carrier about your benefits and claim; that is achieved with a HIPPA release form. If your insurance carrier is Priority Health, please use the Priority Health HIPPA Release Form. For all other carriers, please use the EBS HIPPA release form. The form should be signed by the patient that the claim is in reference to, unless the patient is a minor dependent. We encourage you to read the release form thoroughly, then print off a copy, sign and fax to our office at 231-779-2928, or mail to PO Box 234, Cadillac, MI 49601.
Though our claim resolution turnaround is typically 72 hours or less, (after we receive all pertinent claim information), sometimes more time is needed. We will contact you when we have resolved your claim issue.

Submission of Claims
